Osteochondral Loose Bodies in Joints

Abstract
Authors reviewed 15 joints having the osteochondral loose bodies in 14 patients which were managed during 7 years since 1979 at the Department of Orthopaedic Surgery, Fatima Hospital and the following results were obtained.
1. The cases were 9 males and 6 females with ages from 8 years to 68 years, mostly in third, fourth and fifth decades.
2. The involved joints were 8 knees, 4 elbows and 3 ankles and most of the joints had pain and various findings such as limited mobility, synovial reaction, locking and palpable nodules.
3. The origins of loose bodies were considered to be the osteoarthritis in 5, osteochondritis dissecans in 4 and synovial chondromatosis in 4 cases but the other 2 cases were uncertain.
4. Most loose bodies were demonstrated in simple X-ray but their location in joint cavity was able to confirm by arthrography and air arthrography was considered to be very effective for diagnosis.
5. Open arthrotomy was very effective for removal of the loose bodies completely as well as the additional procedures for the combined pathology which were considered very important to have good result.
6. The loose bodies revealed necrotic bone with osteoclastic resorption on the surface and alive cartilage with successive growth and degenerative changes.
